Dear Patients,
As many of you will be aware, Toftwood Medical Centre is closing on 31 March and patients are being transferred to either Orchard Surgery or Theatre Royal Surgery, where they will receive care from 1 April.
Thank you for your patience and understanding while we have been working hard with all three surgeries to safely transfer care and patient records.
Now we can confirm the patient lists for each surgery, we have written and submitted letters to be posted on Monday 17 March to every Toftwood patient to confirm their new practice. These letters are being posted on Monday 17 March and every patient should receive a letter in the post next week.
The letters will contain information patients need to support with this change, including how to book appointments and order prescriptions.
We ask that current Toftwood patients continue to use Toftwood Medical Centre until the last day it is open on 31 March and that patients don’t seek to register anywhere else between 28 March and 1 May 2025. After this date, if you would like to transfer to another surgery and live in its catchment area, please check their website for more details.
