Travel Vaccinations

If you require vaccinations relating to foreign travel then you need to make an appointment with the practice nurse who will be able to administer these.  When calling the surgery please make the receptionist aware that you need to make an appointment for travel vaccinations, they will then take the details of your trip including countries and areas in which you are travelling and length of your stay so that we can determine which vaccinations needs to be administered.  We ask that you look on the NaTHNaC website before calling to determine whether you need any vaccinations.  If you feel that you do please do not hesitate to contact us.

It is important to make this appointment as soon as possible - you will need to have the jabs at least two weeks before you travel so calling in plenty of time to allow us to book you an appointment is essential.

We are unable to administer some vaccinations that are not covered by the services provided by the NHS.  The receptionist/nurse will be able to make you aware at the time of booking and will be able to signpost you in the direction of a clinic that can provide these vaccines.  Please note that there will be a charge for visiting a private travel clinic.

Non NHS Services

Theatre Royal Surgery offers a range of non NHS services.  There is a fee payable and some of these will be subject to a payment of VAT at the current rate.  Payment in full is payable by credit/debit card or postal order and we will be unable to release the document before it is paid for.

Reports and letters requested will be due for payment even if the patient later cancels the request.  The reason for this is that we have had numerous instances of patients ordering (sometimes complex) reports and later telling us that they no longer need the letter.  This means wasting valuable time for the GP and our secretarial team.

Medical Examinations and Reports

We are able to undertake various medical examinations and complete reports including: HGV, PSV and taxi driver, sports fitness, pre-employment, elderly driver (full examination) and accident or sickness insurance claims (where medication examination is required).

DVLA D4 Medical Examinations - Please be aware that patients attending for HGV medicals are now required to being in photographic identifications and a urine sample.  You may be asked to see an optician/optometrist to have your visual acuity and glasses prescription checked.

Certificates of Various Types and Reports

We can provide general letters or certificates of incapacity, private medical insurance claims or accident insurance (there are three levels of report which will incur different charges depending on the amount of work and information required from the doctor, this will be calculated at the time of completion of the form), bus pass verification, local authority report/certification and child minder health declarations.

If you need documentation for travel we are able to provide this including certificate/letter confirming fitness to travel or freedom of infection, exam and certificate for fitness to travel or freedom of infection, holiday cancellation report/certificate (and if required examination for holiday cancellation forms) or private prescriptions.
