Sick Certificates 

You do not require a doctor's sickness certificate for illnesses lasting seven days or less.  If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of Statutory Sick Pay.

You can request a sick note from the surgery however please be aware that you will need to have been seen by a member of clinical staff at the surgery during your period of illness or provide a hospital letter stating the details of your illness.

We are unable to issue a certificate in advance so please do not call before the end date of your previous certificate and we cannot issue until after the seven day period of self-certification.

If you are able to go to work with amended duties or reduced hours please make this clear when requesting as the GP can issue a note for a phased return.
